First Visit

What to expect on your First Visit

Your schedule is important to us, and keeping the office running on time is always a top priority too. When scheduling your visit, our team members will do everything they can to pinpoint a time that is most convenient for you.

Please bring the following things to our Ponte Vedra Beach, FL location when you arrive for the first time:

  • Driver’s License

  • Dental and/or Medical Insurance Card

  • Your referral slip from your dentist, orthodontist, or physician

  • X-rays if applicable

  • A list of medications you are presently taking

  • Your finished new patient forms (if you’ve chosen to complete them ahead of time)

  • Any relevant medical/dental history

  • Any questions you have!
